- Make an Impact! The Home Health Social Worker provides psychosocial support and counseling for patients receiving home health services. The social worker coordinates psychosocial and community resource needs in collaboration with the interdisciplinary team of nurses, therapists, dietitians, and home health aides. The Home Health Social Worker demonstrates critical thinking, analysis, and clinical judgment in all aspects of care delivery for patients and families experiencing acute and chronic illness. This includes addressing the social, emotional, financial, and spiritual needs of patients and families in order to improve quality of life and home safety.

Position: Social Worker, Home Health
Shift: Full Time Day Shift; 8:30 am to 5:00 pm; Monday through Friday
Experience:
One year social work experience in health care setting preferred. Home Health or other community experience preferred.
Education:
Master's Degree from an accredited School of Social Work.
Licensures/Certifications:
1. Current licensed Social Worker in the state of Pennsylvania required.
2. Valid driver's license and automobile insurance.
3. Current CPR certification.
