- Make an Impact! The Manager of Ambulatory Care Management and Coordination is responsible for the program development and implementation of Care Management and Coordination programs which include Transition of Care, Longitudinal Care Management, Hospital to Home, Medication Adherence and other initiatives to support high risk patients at Main Line HealthCare. This role ensures that the services provided by Ambulatory Care Management RNs for MLHC providers is aligned with the strategic initiatives set by MLHC; and is responsive to the changing healthcare and financial needs of MLH, MLHC providers, patients and administration as well as payers/payer programs. This position provides the direct management of the RN team and oversees the day to day operations of all programs using key performance metrics to determine overall program and staff effectiveness.

Position: Manager, Ambulatory Care Management (RN)
Shift: Monday–Friday; day shift
Experience:
1. Required: 4 years clinical experience in various settings serving adult populations
2. Required: 2 years providing care coordination
3. Payor and value-based contracts experience preferred
4. Working knowledge of payor quality programs including STARS and HEDIS measures.
Education:
1. Bachelors Degree in Nursing (BSN)
Licensure:
Current licensure in the state of PA as a Registered Nurse (RN) required.
