Be a role model, mentor, clinical coach, and a consultant. The Diabetes Care and Education Specialist is responsible for promoting an optimal level of health in medical and surgical inpatients with diabetes. Multidisciplinary education and support are shared to enhance the culture of safety, quality, and coordination of patient care. Utilizing adult learning principles, the Diabetes Care and Education Specialist demonstrates knowledge and skills necessary to coach and support nursing staff in chronic disease states, behavior modification, and key survival skills to equip persons with diabetes the skill set they need to safely self-manage diabetes. In addition, the Diabetes Care and Education Specialist will work with the interprofessional team to support team training initiatives. The Diabetes Care and Education Specialist works collaboratively with other departments to ensure quality patient care and satisfaction. Functions as a leader and facilitator in shared decision-making initiatives at the clinical, departmental, and system levels, resulting in a superior patient experience.

Position: Diabetic Care Coordinator - Lankenau Medical Center
Shift: 36 hours per week 8-hour shifts [ 8am or 9am start time} No weekend No holidays


Experience:
1. Minimum of three years nursing experience in related field required.

2. Demonstrate knowledge of teaching and management principles


Education:
1. Graduation from an accredited school of professional nursing.

2. BSN required. MSN preferred.


Licensures/Certifications:
1. A current RN license to practice in Pennsylvania.

2. Certified Diabetes Care and Education Specialist or willingness to begin qualifications and academic studies for the CDCES examination.
