As an Ambulatory RN, you will play a key role in supporting patients through telephone triage, clinical assessment, medication management, education, and follow-up care that helps improve outcomes and patient confidence.
 

·       Get Involved! Work closely with cardiologists, advanced practice providers, medical assistants, and office staff to support efficient patient care in a busy ambulatory cardiology setting. You will assist with care coordination, symptom calls, medication refills, anticoagulation support, test follow-up, and patient education related to cardiac conditions and treatment plans.

Position: Registered Nurse RN – Ambulatory, Lankenau Heart Group
Shift: Full-time, Days, Monday through Fridays- No Weekends or holidays.

Travel: Travel required up to 6x per month to Paoli, Bryn Mawr and Riddle Hospital locations.

Experience: 
Preferred:
1. Experience in ambulatory cardiology, clinical nursing, specialty practice, or physician office setting. CT Surgery preferred.
2. Strong telephone triage skills, including the ability to assess patient symptoms, provide appropriate guidance, and escalate concerns as needed.
3. Experience with medication refills, patient education, and follow-up related to cardiac testing, procedures, and treatment plans.
4. Knowledge of insurance information, prior authorization workflows, and coordination of care within an outpatient setting preferred.
 

Primary Roles & Responsibilities

Clinical Communication & Triage

  • Manage in-basket workflows including MyChart messages, provider communications, and care coordination tasks
  • Perform telephone triage, assess symptoms, provide clinical guidance, and escalate concerns as needed
  • Review patient charts to support clinical decision-making and follow-up
  • Communicate test results, care plan updates, and provider recommendations to patients

Medication & Orders Management

  • Process prescription refills and support anticoagulation management
  • Place and coordinate diagnostic testing orders and communicate plans with patients and care team

Care Coordination & Follow-Up

  • Coordinate care across outpatient and inpatient settings including ED, lab, and admissions
  • Conduct pre- and post-operative calls, outreach, and ongoing follow-up care
  • Review and manage home care orders and ensure accurate documentation

Clinic & Satellite Support

  • Support patient care at main clinic and satellite locations (Bryn Mawr, Riddle, Paoli)
  • Perform rooming duties (vitals, EKGs, KCCQ, EFTs), patient education, and documentation
  • Maintain clinic readiness, supplies, and assist with patient flow

Documentation & Administrative Support

  • Provide patient education on diagnoses, procedures, and treatment plans
  • Complete FMLA paperwork, patient letters, and maintain accurate medical records
  • Assist with census tracking and management of patient lists

Chart Preparation & Team Support

  • Prepare and review charts, confirm testing, update problem lists, and assist with STS scoring
  • Support providers with procedures, wound care, and clinic operations
  • Assist with rooming, reassessment of vital signs, and patient transitions within the facility

Education: 
Graduate of an accredited ASN or BSN nursing program.

Licensures/Certifications: 
1. Current Pennsylvania RN license.
2. Provider BLS through the American Heart Association or American Red Cross required.
