•  Make an Impact! The Weekend Care Team Leader is responsible for the coordination of client care activities among home health disciplines on weekends and evenings. The Weekend Care Team Leader ensures ongoing communication with providers to assure appropriate management of the client’s care

Position: Weekend Care Team Leader; $52.00/hour set rate
Shift: Days; 2 out of 4 weekends; Hours are from 8:00/8:30am - 5:00 pm, with 1 Friday evening requirement per month from 5:00 - 9:00 pm

Experience:
Home Care Nurse experience is required. 3-5 year(s) of recent Home Care Nurse experience is preferred

Education:
1. RN Diploma, ASN required.
2. BSN preferred.

Licensures/Certifications:
1. PA RN license.
2. Provider BLS (American Heart Assoc/American Red Cross) required.

Requirements:

This position requires you to wear a respirator as part of the job requirement.
